Required Information

Please have your LiveScan Pre-Registration form completed upon your arrival.
If you are requesting a background check for employment or licensing purposes, you must have all documentation from the requestor . This includes the agency name, position applied for and authorization number & type of background (Federal & State or State only.)
If your background check is being sent to a Government Agency you may also need an ORI number.
You must bring a valid form of Government Identification with a photo such as a driver’s license or State Issued identification card.

**Please have a completed LiveScan pre-registration applicant form to include the Authorization Number and ORI Number at the time of your appointment, or we may be unable to process your fingerprint request**
